please advise ,mentioned in 4.1 that this terminolgies apply only to surface and not to internal defect, internal here mean internal surface of the valve or mean internal solid metal

There are more stringent tests than visual for internal, RT, PT, etc. If you have these issues on the inside surface of a valve, then you have a major problem.
