ASME VIII nozzle calc - needs to be reviewed

I have created a ASME VIII, Div 1, sections UG-37 and UG-45 nozzle calculation based on the example in the CASTI Guidebook to ASME Section VIII Div.1 - Pressure Vessels, 4th edition, example 9.1. There are a couple of sections in that example that dont seem to match up with formulas in ASME VIII. I highlighted these in red. If anyone with experience in ASME VIII nozzle design would have a look and offer their advice, I would be grateful.
