How could hazop have helped to to prevent the bhopal disaster

I am writing an essay about this issue. Can somebody give me ideas on how the hazop method could have helped with the bhopal?

I am also writing about fault tree analysis, if you could tell me how it could also prevent the bhopal disaster i would appreciate it.

HAZOP on the new designs could prevent water to enter MIC tank. refer to Appendix 5 in Lee's Loss Prevention Volume 3.
