Safety Observation Scheme/Card/Leaflet

I need to roll out a program where employees can spot hazards & put up as safety observation in a form of leaflet/card etc. Appreciate if you have any of these samples do share with me.

I understand big petroleum companies like Shell, ExxonMobil, Chevron they do have this type of scheme.
