How to write the Report for the Repair Work - Heat Exchanger

Dear Friend,
I again need some help. Recently i had repaired one Heat Exchanger ( Air Cooler Type). We found a problem caused by the Thermal Expansion or vibration of the tubes causing the damage of the Tube- Tubesheet weld get worn out and the Tube material has been lost about few mm at the end portion. Finally we had Plugged the tubes to avoid the problem. I have asked to write a report for this work. i do not know whether their is any format or form as per ASME or NBIC, Our company has U, S, & R Stamp.

So my dear Friends, Please help me in this problem, also if some one had any sample please give to me also. Also please suggest any other method of repairing the tubes.
