Hy Folks!

I need to set up a template for a valve maintenance report. It should contain the main properties of the valve (Like standard, sealing type, SIL spec, etc.).

Further the contractor who is performing the maintenance shall check and add their comments.

Could somebody provide me with a template of such a report?
