Form 3 - Summary Of HSE Management System (HSE-MS)

1. Tenderer shall provide outline of its’ HSE-MS which shall sequentially address all the requirements set forth in SECTION 9.9.
2. Tenderer shall provide an organization chart showing the relationship of the proposed staff shown on form 4 of SECTION 6
3. Tenderer shall provide information that supports the Contractors competency to conduct Hazops/HEMP. Tenderer shall provide the procedure about Hazops review, HEMP review, Plot Plan review and Constructability review, and provide the information about schedule and location of all safety review. This information can be provided during direct negotiation.
4. Tenderer shall submit its HSE Policy and commitments to HSE.
5. Tenderer shall provide the following information in HSE for the three most recent years:

Incident shall include any spills or discharges that required reporting to any government body or violation of any permit or consent decree violations.
