Recently I received the final report of the CSB regarding the accident that occurred at Valero's Refinery in USA on 2007. I want to share with you this important issue in order to you take care in some little changes in operations that may not be a risk for a period of time, but if we do not think clearly as integral engineers, they might represent a tragedy in the future. Even the API standard doesn't consider specifically one of the basic causes of this accident.
